Step 2 – Now fill out your purchase date, time of visit and the code on your receipt. When finished, fill out your email address and press ‘Next’.

Step 3 – On this page, rate your overall satisfaction with the service you received at the store.

Step 4 – Now answer some general questions based on your experience.

Step 5 – On the final page fill out your personal information. When you’ve finished submitting all the information, you are eligible to win the $100 gift card.
